The Centers for Medicare & Medicaid Services (CMS) has released final Merit-based Incentive Payment System (MIPS) scores and performance feedback for the 2025 performance year, opening the targeted review period for eligible clinicians and group practices that believe their scores were calculated incorrectly. The 2025 final scores will determine MIPS payment adjustments applied to Medicare Part B claims in 2027.

CMS provides approximately 60 days for clinicians and practices to request a targeted review. According to the American Medical Association (AMA), the review period opened with the release of final scores and will close 30 days after CMS releases the corresponding 2027 MIPS payment adjustments, which are expected in approximately 1 month.

The AMA notes that circumstances that may warrant a targeted review include data submitted under an incorrect Taxpayer Identification Number or National Provider Identifier, an incorrect payment adjustment for a clinician with Qualifying Alternative Payment Model Participant status, or failure to automatically reweight performance categories for clinicians who qualified for an extreme and uncontrollable circumstances exception. Supporting documentation may be required.

The American Academy of Ophthalmology alerted ophthalmologists to the release September 24, emphasizing the importance of reviewing final scores and requesting a targeted review when needed because of the potential impact on 2027 payments.

Practices can view their final scores and performance feedback and submit targeted review requests through the CMS Quality Payment Program website.
